Proof of Residency is required and must be provided by each parent to create a new Hockey Canada Player Profile. You must attach a scan or picture of your Driver’s License.

Your relationship to the Player?

You understand and agree that the information above may be used to create a Hockey Canada Players Profile, if your child does not already have one, or to transfer their existing Hockey Canada Player Profile in to the Oakville Rangers system. Players are only to have one profile with Hockey Canada – that profile will stay with them anywhere they play within Canada. To the best of your knowledge your child does not already have a Hockey Canada Profile.

Providing false or incomplete information may result in delays in completing your registration and could result in the Player being ineligible to play. You agree that the above information is accurate and completed in full.
