Please read for information on the 2017/2018 LOR PERMISSION TO SKATE FORMS

LOR PERMISSION TO SKATE FORMS:  Please be advised that as per Ontario Hockey Federation (OHF) Rules and Regulations the Minor Oaks Hockey Association is not permitted to provide Permission to Skate (PTS) forms until the completion of the OHF Championships. This form is necessary for 2017/18 Oakville hockey players that are Lake Ontario Region (LOR) residents wishing to try out for other LOR 2017/18 teams at Minor Bantam and above (PTS forms will not be used for Peewee and below). The first date PTS forms will be available is Monday, April 10th, 2017. PTS forms are provided for players from teams that are part of the Lake Ontario Region (LOR) that wish to try out for other teams in the LOR. For more information on the Lake Ontario Region please visit the OHF website. MOHA will begin to accept requests via the online system commencing March 28, 2017, please see below for instructions.

OHF AAA HOCKEY WAIVER:  The OHF AAA waiver is necessary for those players wishing to attend tryouts outside of the Lake Ontario Region (LOR).  Please be advised that as per Ontario Hockey Federation (OHF) Rules and Regulations the Minor Oaks Hockey Association is not permitted to provide waivers until the completion of the OHF Championships. The first date OHF waivers will be available is Monday, April 10, 2017 and MOHA will accept requests by email only to [email protected] starting March 28, 2017. Please include your child’s name, date of birth, address, team played on in 2016/2017 and specify if the player is a goalie or skater.  Please include the name of the team that the player requires the waiver for. Permission for this waiver must be approved via MOHA by the respective coach. Waivers will not be available for pick up until April 10, 2017 at the earliest. You will be contacted via email when your waiver is ready for pick up and at the time of pick up both the player and a parent must be present to sign the waiver.

Online Request for Permission to Skate Forms

Requests for Permission to Skate Forms is done via the online system.  Please follow the instructions below in order to access the system and submit your requests.

Go to the website

You will have to first create an account to begin using the website.  Click on “Register” on the top right of the screen and then complete the Sign Up information.  Please ensure that your email address is correct and that you take note of your password.  You will need this information in order to log in again once you receive notification that your request has been approved.

Once you are registered and logged in to the site, select “Minor Oaks Hockey Association” from the list.  If you choose any other association they will reject your request and you will have to resubmit to us.

There are two forms currently available on the site, the Lake Ontario Region Permission to Skate and the OMHA Junior Permission to Skate.  The Midget AAA forms are not yet available but will be posted once they are, closer to August.  Select the form you wish to request and complete the questions on the screen ensuring that all information is accurate and then click “Submit”.  If you have more than one team you wish to tryout with, you can then continue to enter the next one and submit again. Once all your requests have been submitted you can Log Off.

You will receive an email confirmation that your request(s) have been submitted and an email will be sent to MOHA advising us of the request(s).

MOHA will access the system and review all the PTS requests.  If your request has been approved, you will be sent an email advising you and then you can go back into the same website to download and print the completed PTS form.  If your request has been denied, you will also receive an email with an explanation as to why it was rejected.

Once the PTS has been approved, you can sign back in to and then select “My Forms” on the top right of the screen.  Your forms submitted will be listed there with the option to “Download”.  Click on that and you will be able to print the signed form from your location rather than having to come to the office to pick it up.

Please allow for at least one business day for notification of approvals.
