Please click here to review the OHF LOR Player Movement Regulations. As an Oakville Resident, you reside within the Lake Ontario Region (LOR). If you are trying out for an LOR Team outside of your Home Center you will first need to obtain a Permission To Skate. If you you are trying out for a AAA Team outside your Home Center you will require an OHF AAA Hockey Waiver.

As per Ontario Hockey Federation (OHF) Rules and Regulations the ORHC is not permitted to provide PTS Forms or Waivers for the 2018/19 season until April 16th, 2018

LOR Lake Ontario Region What is the LOR Lake Ontario Region

LOR stands for Lake Ontario Region. The LOR encompasses the whole of the GTHL (Toronto, Markham, Mississauga, and Vaughan) and the OMHA Associations/Centres (Oakville, Brampton, Richmond Hill, Markham, Ajax and Pickering). It has been in existence since the 1997-1998 season, to address the unique geographical area in and around the Greater Toronto Area. If you live within the LOR you are governed by the LOR Regulations. LOR Permission To Skate (PTS) Forms

  • You are trying out for a Minor Bantam or above team
  • You want to try out for a team within the Lake Ontario Region (LOR) outside of your Home Center (Vaughn, for example)

If you meet the criteria listed above you will require a Permission To Skate before you attend the Tryout. As per Ontario Hockey Federation (OHF) Rules and Regulations no Association is permitted to provide Permission to Skate (PTS) forms until the completion of the OHF Championships. For the 2018/19 Tryouts, the first date PTS forms will be available is Monday, April 16th, 2018. PTS forms are provided for players from teams that are part of the Lake Ontario Region (LOR) that wish to try out for other teams in the LOR.

Follow the instructions below to request a PTS:

  1. Go to https://www.minorhockeyforms.com/Organizations/1/Oakville-Rangers-Hockey-Club
  2. Log in to your account
    • If you have forgotten your password click on Forgot Password
    • If you do not have an account click on Register As A New User
  3. Select Oakville Rangers Hockey Club from the list of Hockey Clubs
  4. Locate the 2018-2019 Lake Ontario Region (LOR) Permission To Skate form and click on Go To Form
  5. Fill out the form then click on Submit

You cannot change your email address on Minor Hockey Forms!
If your email address on Minor Hockey Forms is no longer valid create a new account

Please allow 1-2 business days for your PTS Request to be processed. Rush requests will not be accepted – do not wait for the day of the tryout to request your PTS. Once approved you will receive an email notification along with a link for you to download and print the form. You must print the approved PTS and bring it with you to the try out.




OHF AAA Hockey Waiver
  • You are an Oakville Resident
  • You want to try out for a AAA Team outside your Home Center

If you meet the criteria listed above you will require an OHF AAA Waiver before you attend the Tryout. As per Ontario Hockey Federation (OHF) Rules and Regulations no Association is permitted to provide Permission to Skate (PTS) forms until the completion of the OHF Championships. For the 2018/19 Tryouts, the first date PTS forms will be available is Monday, April 16th, 2018.

To request an OHF AAA F1 Waiver, please email Melissa Wolk at [email protected]. Please be sure your email includes the following information:

  • Player’s Full Name
  • Date of Birth
  • Home Address
  • Phone Number
  • What team did they play for last season?
  • Which team are they signing with?
  • Are they a Goalie or a Skater?

 

The ORHC will accept Waiver Requests by email only

Once approved, you will be contacted via email to make an appointment to pick up the Waiver at the ORHC Office. Both the player and a parent must be present to sign the waiver when you come to pick it up.
