Please click here to review the OHF Major Midget AAA Regulations. If you are trying out for a Midget AAA Team outside of your Home AAA Center you will first need to obtain a Permission To Skate. If you have been selected to a Midget AAA Team outside of your Home AAA Center you will require a Midget AAA Release.

As per Ontario Hockey Federation (OHF) Rules and Regulations the ORHC is not permitted to provide PTS Forms or Releases for the 2018/19 season until after August 23rd.

LOR Lake Ontario Region What is the LOR Lake Ontario Region

LOR stands for Lake Ontario Region. The LOR encompasses the whole of the GTHL (Toronto, Markham, Mississauga, and Vaughan) and the OMHA Associations/Centres (Oakville, Brampton, Richmond Hill, Markham, Ajax and Pickering). It has been in existence since the 1997-1998 season, to address the unique geographical area in and around the Greater Toronto Area. If you live within the LOR you are governed by the LOR Regulations. How To Request A Permission To Skate (PTS) Form

  • You are trying out for a Midget AAA Team
  • You are an Oakville resident (you live and/or go to school in Oakville)
  • You want to try out for a team within the Lake Ontario Region (LOR) outside of your Home Center (for example, a Toronto resident trying out in Oakville or an Oakville resident trying out in Ajax)

If you meet the criteria listed above you will require a Permission To Skate before you attend the Tryout. As per Ontario Hockey Federation (OHF) Rules and Regulations no Association is permitted to provide Permission to Skate (PTS) forms until on or after August 23rd. Please follow the instructions below to request a PTS:

  1. Go to http://minorhockeyforms.com/Account/Login
  2. Log in to your account
    • If you have forgotten your password click on Forgot Password
    • If you do not have an account click on Register As A New User
  3. Select Minor Oaks Hockey Association from the list of Hockey Clubs
  4. Locate the OMHA Major Midget Player Tryout Permission Form form and click on Go To Form
  5. Fill out the form then click on Submit

You cannot change your email address on Minor Hockey Forms!
If your email address on Minor Hockey Forms is no longer valid create a new account

Please allow 1-2 business days for your PTS Request to be processed. Rush requests will not be accepted – do not wait for the day of the tryout to request your PTS. Once approved you will receive an email notification along with a link for you to download and print the form. You must print the approved PTS and bring it with you to the try out.




How To Request A Major Midget AAA Release
  • You are an Oakville Resident
  • You have been selected for a Major Midget AAA Team within the Lake Ontario Region (LOR)

As per Ontario Hockey Federation (OHF) Rules and Regulations no Association is permitted to provide Midget AAA Releases until on or after August 23rd. If you meet the criteria listed above, please email Melissa Wolk at [email protected] to request a Release. Please be sure your email includes the following information:

  • Player’s Full Name
  • Date of Birth
  • Home Address
  • Phone Number
  • What team did they play for last season?
  • Which team are they signing with?
  • Are they a Goalie or a Skater?


The ORHC will accept Release Requests by email only
Releases will not be issued unless you have been offered a spot on a team

Permission for this release must be approved via the ORHC by the Midget AAA Coach. Once approved, you will be contacted via email to make an appointment to pick up the Release at the ORHC Office. Both the player and a parent must be present to sign the waiver when you come to pick it up.
